Our clinicians connect with you virtually to help with your care plan, provide timely visits during nights, weekends, and holidays, and resolve any emerging changes in your health.

What Lumina Care does for patients and families
We are a clinical team that provides telehealth care over the phone or through video calls. Most often, we provide care for individuals at a skilled nursing facility, senior living community, or while they are recovering at home from a hospital stay. Wherever we work, we are a seamless extension of your primary care team.

What to expect during a virtual visit
If a Lumina Care visit is scheduled for you, a nurse or staff member will help you connect with a provider. During the visit, our clinician will:
Review your medical history and recent notes
Ask how you are feeling and what has changed
Examine you as needed through video and work with onsite staff for vital signs or other checks
Explain their recommendations and answer your questions
Share a summary with your care team so everyone stays aligned

Lumina Care follows the same privacy rules and standards as your other healthcare providers. Your information is protected under HIPAA, and any virtual visit is documented in your medical record so your full care team can see what was done and why.
