What Lumina Care does for patients and families

We are a clinical team that provides telehealth care over the phone or through video calls. Most often, we provide care for individuals at a skilled nursing facility, senior living community, or while they are recovering at home from a hospital stay. Wherever we work, we are a seamless extension of your primary care team.

What to expect during a virtual visit

If a Lumina Care visit is scheduled for you, a nurse or staff member will help you connect with a provider. During the visit, our clinician will:

  • Review your medical history and recent notes

  • Ask how you are feeling and what has changed

  • Examine you as needed through video and work with onsite staff for vital signs or other checks

  • Explain their recommendations and answer your questions

  • Share a summary with your care team so everyone stays aligned
